Changing Your PCM

Request a new Primary Care Manager (PCM) in two easy steps:

  1. Use TriWest’s Prime Enrollment Opportunities tool to locate a new PCM near you.
  2. Request the PCM change online through the Beneficiary Web Enrollment (BWE) site.

You may also print and complete a PCM Change Form and submit it to TriWest at the address noted on the form.
