Behavioral Health Form Updated

TriWest has recently updated the Preauthorization for Psychological/Neuropsychological Testing form available on www.triwest.com/provider. Updates and improvements to the form include changes aimed at simplifying the form (referring provider contact information is no longer required and multi-part questions have been separated) and ensuring the form arrives at TriWest complete and ready for prior authorization.

The form remains downloadable in Microsoft Word format so providers can type the information directly into the form, then fax it to TriWest. Each question must be answered or the form will be returned to the provider.
