The Provider Directory has been designed to be as user-friendly as possible. The Provider Directory search allows users to provider information both about their location as well as details of the desired provider in order to provide specific results.
- Search for a provider based on the users Zip Code, City and State.
- Search for a provider based on the name of the provider.
- Search for a provider by the name of their medical group.
- Search for hospitals, surgery centers, and ancillary facilities.
Performing a Search: To perform a search within the Provider Directory only a zip code and state are required. However the more information provided, the better the search results. The best results can be achieved by providing a full address along with specifics regarding the type of provider you are searching for.
To Search:
- Enter a State and Zip Code. These are required elements.
- Enter/Select any of the optional fields.
- Click the 'Search Providers' button.
Hints: On the Search page there are a number of optional information items. These items provide additional information that can assist you in searching for a provider.
Viewing Results: After entering in the desired search criteria and clicking the 'Search Providers' button, the search results will be presented. Depending on how specific the search criteria was would determine the number of search results received. At any time after results are displayed, a new search can be performed or clicking on the 'Search Providers' button can refine results.
Results are presented in a html format, much like how file folders look. The first tab displays Military Treatment Facilities (MTFs) that fall within the search criteria entered. The second tab displays individual providers that fall within the search criteria entered.
To switch between MTF & Provider listings click on the labeled tab at the top of the results list.
Based on the number of results found it may be necessary to navigate to other pages to see all the available results. At the top of the results list (directly below the tabs) will be a listing of all the results pages available.
To navigate between results pages:
- Clicking on the linked number will jump to that specific page
- Clicking on > will go to the Next page
- Clicking on < will go to the Previous page
- Clicking on >> will go to the Last page of the results found
- Clicking on << will go to the First page of the results found
Aside from the list view presented after searching, details on the MTF facility and provider are available by clicking on the linked name of the facility or provider. In many cases additional information about the MTF or provider and listed. It is also at this point where corrections to provider information can be suggested.
Reporting errors in Provider Information The on-line Provider Directory is updated every week, however sometimes there may be incorrect or new information about a provider that has not yet been updated on the web site. If you find an error or find that a provider is missing from the directory you can suggest a change by doing the following:
- After searching for a provider, click the link of the particular provider you would like to suggest changes for. The link is the name of the provider.
- On the provider detail page, click the link 'Suggest Changes to This Provider'.
- Follow the directions on the subsequent pages. There will be an option to update the particular provider being viewed or to suggest a provider that is missing from the directory.
Using Driving Directions The Point of Origin is determined by the address information the user provided in the Basic Search or Advanced Search screens.
- Entry of complete address information (City, State AND ZIP Code) will determine the Point of Origin most accurately.
- Entry of City and State only will place the Point of Origin in the center of the city.
- Entry of ZIP Code only will place the Point of Origin in the center of the zip code.
Important note: The distance from the Point of Origin to a Provider's location is based upon the exact longitude and latitude of both points. The distance (mileage) displayed on the search results page is derived "as the crow flies" (a straight line). Therefore, actual driving distances may vary.
The mileage distance displayed on the results is a straight-line distance from the address information entered. It does not take lakes, rivers, roads, etc. into consideration. For an exact distance, please select the driving directions icon.
The Driving Direction Icon is displayed in the last column of a provider's information in the Provider Listings Section of the Search Results screen.
By default, the user is provided with a large-scale map along with turn-by-turn directions. The user has an option to select Display Turn-by-Turn Driving Maps. This option will provide the user with specific maps for each separate instruction. The Printer Friendly version is recommended for printing driving directions because it contains the fewest maps.
