TriWest Prior Authorization List Codes Updated

TriWest Healthcare Alliance has updated the lists of codes requiring prior authorization in our online Prior Authorization List (PAL). Changes effective April 1, 2007, included, but are not limited to, the following:

• Moved biofeedback codes 90911 and 90901 from the Home Health section and to the Physical Therapy section
• Added unlisted code A4335
• Removed code 64450 application of surface (transcutaneous) neurostimulator
• Added surgery codes to the inpatient section
• Added drugs J1438 Etanercept (Enbrel) and Navelbine/vinorelbine (no code)
• Removed drugs J0770, J1675, J9225, J2430, J3487 and Gliadel Wafer
• Changed the spine surgery section to add several codes and delete several other codes
• Codes 11981, 11982, 11983 were removed from the Implantable Pump list under the Surgery section
