If you would like for TriWest to send you our privacy policies, to update, correct, delete your personal information, or to exercise any other state-specific personal data rights available to you please complete the Consumer Privacy Request Form.

The information you provide on this form will be used to a) Verify your identity and/or the identity of an authorized agent; b) to investigate and determine if TriWest has collected any Personal Information about you or a party you are authorized to represent; c) to determine what action can be completed by TriWest; and c) to investigate waste, fraud and abuse, security and other privacy concerns.
